A toilet with a sink and a soap dispenser but no soap. At a hospital. 
Nurses, and medical staff hierarchically below them, use said toilets and go on to care for patients.
Don't know what the doctors do. May be they have separate doctor-worthy toilets which include soap. Or perhaps they don't need to use toilets because they are gods? 
@smurthys I believe my mom, a retired RN, would say the doctors most definitely think they are gods.
